I don't think you do need the schools stamp on it.Try talking to the assistant at PO for help or ring up the no on form for help with this but as long as you have birth/passport this should be enough.

Most parents would want a CRB check to make sure you had no criminal convictions/warnings & also a recognised First Aid Certificate-American Red Cross etc.
